A nice idea in theory, but I can't see this happening as is with the 86s frequency and stand requirement. There won't be enough stand space for 3/4 additional buses.
Maybe a situation like a one in three running to the hospital could happen?

Rough calculations indicate 36 buses per hour traverse between Romford Station and Queen's Hospital. There's no way these buses are all heaving towards the hospital as well.
The 86 would need at least two buses added to its PVR for an extension to Queen's Hospital, and that's before we even think about stand space. Complete non-starter of a proposal.

5,128,175,193,365,496,498 all running between Romford Station and Queen's Hospital.
Night Bus 128 and 365 so pass through this Queen's Hospital area. N15 is nearby.
No need for route 86 extension to Queen's Hospital.
