I think there was a suggestion to make the 174 a 24hr service, with the N86 being cut back to Romford (becoming a 24 hr service essentially). I could certainly see this being useful as Dagenham needs more night routes. Dagenham is a night route wasteland. There are plenty of options on how to improve connections. The 174 is an option to bring into a full night service, but part of me feels like the 173 could also work. King George Hospital doesn’t have a 24 hour route, so perhaps the 173 could be an option. Hornchurch and Upminster could also benefit from a night route, such as a re-routed N86 or a brand new N66. Rainham also has some options. A 24 hour 165 (if the 365 is no longer 24hr) or 287 might be a bit much, so maybe an N238 following the 287 would work, or an N552 as a branch of the N15. Isn’t the hospital closed during night hours? That might be why the hospital doesn’t have a 24hr service. Ik the N15 avoids Queens Hospital unlike the 5. I remember the 145 also used to have a weekend night service, which tfl withdrew because not enough people were using it.

I'm not sure that necessarily justifies a night service, shifts usually start and finish at times when public transport is operating and patients are likely to use taxis.

King George is indeed a 24 hour A&E, it's always been an anomaly that it's not had a night service and people haven't really been fighting for one. Newham Hospital is another one, although the N15 is only a few minutes walk down the road one way with the N551 the other way which might be why that hospital gets away with it.

I would propose the night route variant N277 to be extended to Marble Arch via the 30. I think there is some merit in bringing links such as the current 30 to a night service, maybe at x30 minute intervals. There are other routes that do the Dalston/Hackney to Central links here, the 76/149/243/N38 albeit all different routings but the N277 also takes its own to Angel, and would take its own through Highbury, Fitzrovia, Marylebone which all have some decent night usage.
Another suggestion would be to remove the N242 as a whole and create an N30 that diverts to Homerton Hospital at Hackney Central. The N242 wouldn’t need any replacing - the TCR to Dalston link can be retained by the N38 and the TCR to Hackney link can be retained by the N55 (the most common journeys on this route). Rest of the section has the N8 and other routes are there for the small little journeys (such as 149 for Liv Street to Haggerston etc)

With the proposed N518, one thing I would do is reroute the route in the Northolt Park section, by having it run into South Harrow and then round the bus station like the 398 does. I don't get why it does not already serve South Harrow and this rerouting would not affect the linking of any areas.

another idea is a N37 running all the way to richmond to replace the 337 at night

Given how short the 264(N) presently is compared to other night routes
N264: Tooting bound
Rerouted from Mitcham Fair Green¹ via the 200 through Phipps Bridge and Christchurch Road to Colliers Wood Station² to then loop around via High Street Colliers Wood and Priory Road back to Christchurch Road then via the 152 along Western Road to Mitcham Fair Green to then continue on to Tooting
N264: Croydon bound
Essentially the reverse loop from Mitcham Fair Green via Western Road, Christchurch Road, Priory Road, High Street Colliers Wood, Christchurch Road³, Phipps Bridge, Mitcham Cricketers, Mitcham Fair Green
To summarise Tooting bound buses incorporate a loop via the 200 to Colliers Wood and the 152 back to Mitcham with Croydon bound buses doing so in reverse. None of the present 264 stops are missed out.
¹ - Upper Green East bus stop
² - 152/200's New Malden/Raynes Park bound stop
³ - Colliers Wood Station stop for 152/200 towards Pollards Hill/Mitcham

Always found it shocking that the Dagenham had no night service bar N15 and weekend 145, I once had to walk from Aveley to Martin's Corner in my younger days because the last 372 didn't turn up 😂 (original Blue Triangle days around 07/08) , I think N86 should stay as is , as it is basically a Liz Line replacement over night now could even divert at Gallows and follow 498 to Brentwood and have a N174 and the 248 should be made 24 hour , how Hornchurch and Upminster have no Night Service I don't know the same with the N365 it should extend to Tesco at Rainham at least , But I'm sure with Beam Park coming along some route will get extended and have a 24 hour service maybe even the new SL"66" Via Elm Park

Shifts don’t tend to finish in the middle of the night, there are no visiting hours and let’s be honest if you’re well enough to walk to the bus stop, wait for the bus and then walk to the A&E you can’t be that unwell. If it’s an emergency I’m calling an ambulance or getting an Uber right to the front door. Absolutely zero chance most people would wait for a bus.

St Helier hasn't had one since 2003 and doesn't seem to have had any issues or complaints that I know off so totally agree that demand to a hospital at night is low.

Epsom doesn’t have a night service either and similarly there are complaints I’m aware of. Personally I wouldn’t go to St Helier at night, it’s horrible enough during the day. I’ve always said if I’m conscious I will demand to be taken to St George’s or Kingston.

In fairness that wouldn't be a bad thing anyways to serve Brighton Road.

Could be Hundreds of reasons somebody needs to go A&E or from A&E , not everything has to be blue lights and a ambulance , so yeah I think buses serving hospitals 24/7 is a good thing and makes sense .
